Episode 5 begins with a dinner gathering for the OB-GYN team, where the professors teasingly try to get the residents to reveal the nicknames they’ve given their seniors. I-yeong drunkenly blurts them out: Professor Seo is known as “Twenty Questions” for her barrage of inquiries, and Professor Kong is called “The Screaming Devil” due to her constant shouting. Later, I-yeong gets caught munching on someone else’s snacks, and she discovers it’s Ahn Jeong-won. The next day, hospital life returns to normal. Jae-il mistakenly diagnoses a woman’s stool as a tumor, Sa-bi is disappointed in her gynecology rotation despite praise from Professor Ryu, and Pyo Nam-kyung struggles with the breakup of her long-term relationship, secretly hoping her ex-boyfriend will reach out.
I-yeong, overworked from her intern’s mistakes, loses her temper and yells at them in front of patients. Do-won witnesses this and reprimands I-yeong, which creates tension between them. Later, on her birthday, I-yeong asks Do-won for a birthday wish and makes him promise to say yes to everything she asks while in the elevator. When she admits that she likes him, he says yes, but immediately afterward, he shuts down her hope, telling her it would make things awkward between them.
The following day, I-yeong celebrates her birthday with her family before heading to work. Jae-il feels proud of his diagnoses but unwittingly burdens his mentors with unnecessary calls. Meanwhile, Sa-bi faces a tough situation when a grandmother refuses surgery for a prolapsed uterus due to embarrassment. Sa-bi and Professor Ryu convince the woman to undergo the procedure, framing it as a testament to her life’s sacrifices. Later, I-yeong helps a patient overcome her fear of surgery by shouting her name to wake her after the procedure, keeping a promise she made.
Do-won, still avoiding I-yeong, focuses on helping Dr. Myeong with research. Meanwhile, Jae-il brags about his successful diagnoses to Do-won but gets scolded for overloading his seniors. When the residents finally get some peace, Sa-bi jinxes them, and a Code Blue is announced. One of the mothers from earlier in the episode, who had bonded with another patient, faces a rapid decline, and it becomes clear she won’t survive. I-yeong must tell the woman’s young daughter about her mother’s condition, triggering memories of her own loss. Jeong-won steps in, offering comfort and facilitating a final goodbye between the mother and daughter.
The episode ends with I-yeong experiencing a powerful flashback to her mother’s death, overwhelming her with grief. As she hears her mother’s voice but finds no one there, the emotional weight of her loss becomes clear.
