Resident Playbook Episode 7 kicks off with the interns gossiping about the whirlwind of personalities among the first-year residents. Oh-young is notorious for her temper, Jae-il wears his heart on his sleeve, Nam-kyung is so loud they joke about checking her blood pressure, and Sa-bi remains unbothered and as “undiagnosed” as ever.
Meanwhile, Professors Seo and Ryu set their sights on recruiting intern Tak Gi-on to the OB-GYN department. They inform Nam-kyung that Gi-on admires her, hoping she’ll be extra kind to sway his choice. But the plan quickly runs into trouble when Gi-on confuses two patients with the same name and mistakenly informs Ik-sun that her illness has returned. Though Chi-hong reprimands him, Nam-kyung holds her composure and remains supportive.
Over in the skills lab, the residents practice sutures, with I-yeong standing out for her finesse. Back in the OB-GYN wing, the chaos doesn’t stop. Emergency deliveries disrupt scheduled surgeries, drawing complaints from other departments and leaving the residents to play peacemakers.
A more delicate case arrives in the form of a cancer patient whose son begs the doctors not to disclose her diagnosis, fearing she’ll refuse surgery. The team agrees. Meanwhile, the residents meet up with Do-won for dinner. He offers to treat I-yeong, only to remember he’s heading out for vacation. As his departure nears, multiple professors ask for last-minute help—an unspoken testament to how reliable he’s become.
Despite her efforts, Nam-kyung can’t seem to catch a break. Whether it’s praise from patients or doctors, it all seems to land on Gi-on, making her feel increasingly invisible.
Elsewhere, Sa-bi steps in to resolve a space crunch in the nursery, smoothing over tensions with pediatrics. I-yeong has a heated exchange with the anesthesia team over an urgent delivery, and Jae-il stands firm when a doctor tries to delay a patient’s testing. Eventually, the first-year residents sit down for a much-needed meal and agree: they need to stop apologizing and start asserting themselves.
Do-won leaves for his Taiwan trip—almost. Gi-on, meanwhile, accidentally lets the cancer diagnosis slip to the patient. Distraught, she nearly refuses surgery. Professors Ryu and Nam-kyung apologize, but it’s Gi-on’s honesty that ultimately sways her to proceed.
Just when I-yeong thinks she’s off the clock, an emergency comes in: a foreign patient who speaks neither Korean nor English is in labor. With no other doctors around, Professor Seo urges I-yeong to take the lead. Nervously prepping for surgery, she’s stunned when Do-won rushes in—he missed his flight after oversleeping and came straight to the hospital. Together, they deliver the baby safely, saving both mother and child.
In a quieter subplot, Nam-kyung finally hears from her boyfriend, only to learn he’s at the hospital with his injured mother. Their conversation reveals what’s long been unspoken—they’re done. They part on peaceful terms in a quiet, mutual breakup.
As the episode winds down, I-yeong texts Do-won to meet her on the rooftop. But when she arrives, her colleague from the anesthesiology department beats him to it—and asks her out for dinner. Just then, Do-won appears and catches the scene, silently taking in the moment.
